What you need to bring to all appointments: *90 days proof of income for all household members *Social Security numbers for all household members over age 2 *Birth dates for all household members *A letter/statement from mortgage co. or landlord with mortgage co. or landlord’s name; address; phone number; amount of rent or mortgage; and the letter must state you are behind or need a first month’s rent or you are being evicted or are in foreclosure status

FEMA Rental/Mortgage Assistance: To Qualify: *You must be at $150 of the poverty level *You must be behind on your mortgage/rent or need 1st month rent. · Limit is one month’s rent not to exceed $300 per household per year. ¨ Call Trina Payne the LAST Tuesday of the month at 740-702-7222 Ext. 117.
HOMELESS PREVENTION PROGRAM
Call for appointment, limited funding per month. Foreclosure Assistance: To Qualify: *You must make $25,102 per year or below *Must have a letter of foreclosure to bring with you *Limit is $500 per household per year
Homeless Prevention Rent: To Qualify: *You must make $17,570 per year or below *Must have an eviction notice to bring with you *Limit $400 per household per year
Deposit: ONLY when being evicted from current residence to qualify. *You must be at 125% of the poverty level *You must need a first month deposit & bring a letter from a landlord stating this with the amount *Limit $400 per household per year
